- Induction Dues and Fees for new Members (Mail)
- Tripod Event Form (minimum of 3 for semester schools and 2 for quarter schools) (CAP) Please note: By May 2022 for semester schools and June 2022 for quarter schools, each Chapter must submit Tripod forms that have at least two elements of each leg of the Tripod (2 scholarship, 2 leadership/service, 2 fellowship). Events may incorporate multiple areas of the Tripod. For instance, an event may have a scholarship element and a service element. These will also be used in consideration for 2022 National awards.
